Thomas C. Pomfret, PharmD, MPH, BCPS is a Clinical Team Lead and the Postgraduate Year One (PGY-1) Managed Care Pharmacy Residency Program Director at ForHealth Consulting at UMass Chan Medical School. With over 18 years of managed care pharmacy experience, he has been involved in the creation and implementation of clinical pharmacy programs as well as formulary and utilization management strategies for various Medicaid and commercial-based health plans. Dr. Pomfret has been the Managed Care Pharmacy Residency Program Director since 2014. He co-leads the health equity pharmacy efforts for the Massachusetts Medicaid (MassHealth) Pharmacy Program. He is also the Chair of the Cultural Diversity Committee at ForHealth Consulting, which supports diversity, equity, and inclusion (DEI) efforts across the workforce, develops and administers educational programs and trainings, and advances DEI strategic efforts and goals for the organization.
Dr. Pomfret has served as a member of the AMCP Health Disparities Advisory Group, the AMCP Professional Practice Committee, is an American Society of Health-Systems Pharmacists (ASHP) practitioner surveyor for managed care residency programs, and is the AMCP representative to the ASHP Commission on Credentialing. He was awarded the AMCP Residency Program Director of the Year award in 2022 and the AMCP Individual Contribution award in 2024. Dr. Pomfret received his PharmD degree from the Massachusetts College of Pharmacy & Health Sciences (Boston) and a Master of Public Health (Health Policy & Administration) degree from the University of Massachusetts – Amherst. He is a Board Certified Pharmacotherapy Specialist.
